Home from Hospital

• Help with home adaptations and equipment such as key safes, handrails or arranging for a deep clean
• Make sure that you have food and other essential supplies following your stay in hospital
• Support to return home following a stay in hospital including pre-planned procedures
• Access other services i.e. health and social care, meals on wheels, home help services
• Make arrangements to collect essential items from your home. i.e. (glasses/post)

• Help find opportunities to overcome loneliness and isolation
• Access health and social care services as well as community health & wellbeing services

• Deal with rent arrears and other money problems and access specialist debt advice
• Claim relevant welfare and other benefits and/or grants

• Make choices about your housing situation, identifying your housing options as your needs change and referring to services to support you
• Help you settle successfully in your new home; i.e. finding furniture, household items and setting up utility accounts

We also provide a dedicated support service for unpaid carers with family, friends or neighbours in any of our local hospitals.

• Clarify and understand hospital services and processes
• Be a point of contact with the hospital to help you be up to date and involved with a loved ones care whilst in hospital
• Help to liaise with the discharge team to arrange care of a loved one before and after discharge from hospital
