Do you need some help while you are in hospital and support to help get things in place for when you return home?
What we do
Whether you are going into hospital or currently in hospital, we can support you and make sure you're able to return home safely.
We work in close partnership with local hospitals and other organisations to deliver our support.
This service is open to anyone in the area. You don't need to live in an Alliance home - we can provide the expert support you need.
The service is free and funded by North Somerset Council for people over 18.
